A Retrospective Study for the Significance of Barium Enema in Suspicious Acute Appendicitis


Abstract
Acute appendicitis is the most common acute surgical condition of the abdomen. Despite its frequency and its well known classic findings, the frequent atypical findings present diagnostic difficulties. Barium enema is proven to be safe and useful
in
confirming the diagnosis of the suspticous appendicitis and reducing the negative surgical exploration.
We performed 112 barium enema in suspicious appendicitis during the period from january 1990 to December 1992. Postive findings, of barium enema were 57 cases. All cases were explorated and 53 cases were revealed acute appendicitis by
histopathologic
findings. Suspected positive findings of barium enema were 6 cases and 4 cases were explorated and 2 cases were acute appendicitis but 2 cases were not. Negative findings of barium enema were 49 cases. Among them, 6 cases were explorated. And
only

cases proved to be acute appendicitis.
The result of barium enema were 92.0% in sensitivity 97.9% in specifity, 7.0% in false positive, and 2.0% in false negative. However there was no barium leakage and/or barium appendicitis. We concluded that the barium enema was valuable and safe
diagnostic procedure and recommend its performance in all patients who pose a diagnostic dilemma.(Korean J Gastroenterol 1994 ; 26 : 473-481)
